Make a Difference in Your Rehospitalization Rates Virtual, Live Training

Historically, one-fifth of Medicare beneficiaries are re-hospitalized within 30 days of discharge, costing centers roughly 17.4 billion dollars in unplanned rehospitalizations. Rehospitalization rates are linked to increased medication errors and hospital-acquired infections, among many other challenges. FHCA's Make a Difference in Your Rehospitalization Rates is a two-hour virtual, live training on April 10, 2025, from 1:30 to 3:30 p.m. EST featuring Amina Dubuisson, Vice President of Clinical Services for Ventura Services. Attendees will learn best practices to catch the warning signs for high-risk residents, perform rounds with rehospitalization risks in mind, engage the interdisciplinary team in reducing return to hospital rates, and other tangible steps to make a positive impact on resident care.

Learner Objectives

Upon completion, participants will be able to:

  • Understand the current return to hospital rates in Florida and review the lasting impact on resident care.
  • Discuss how to target high risk residents and intervene early.
  • Explore ways to reduce rehospitalizations, including an overview of the advantages of the Interact tool.

Amina Dubuisson

Amina Dubuisson has been a nurse for over 24 years. She earned an Associate of Science degree in nursing from Miami Dade College and become a Registered Nurse. She received her Masters of Science in Nursing, Business Administration and Healthcare Management from the University of Phoenix; and her Doctorate in Nursing Practice from Chamberlain University. She is currently the Vice-President of Clinical Services at Ventura Services of Florida, Inc. She is the Vice Co-Chair of the Senior Clinicians Council from the Florida Health Care Association. She received the Florida Health Care Association Rising Star in Nursing Award and was recognized as an emerging nurse leader who has demonstrated excellence in nursing and person-centered care practices. She was recognized by Greystone Healthcare Management for her volunteer works in the community. Amina's biggest achievement has been her ability to give back to the community through volunteering and providing care to the underserved population through various Medical Missions and Health Fairs. Every year, she travels to Haiti at least twice in Medical Missions. She believes that everyone has a purpose in life and her purpose is to help people.
