Partnering with HQI-QIO Will Improve Quality Outcomes

Includes a Live Web Event on 03/10/2026 at 1:00 PM (EDT)

Live Webinar: March 10, 2026
1:00 p.m. EST

Long term care providers are caring for residents with increasingly complex medical, functional, and cognitive needs, making early recognition of decline essential. In this session, Health Quality Innovators (HQI), the new CMS Quality Improvement Organization, will provide a practical, systems‑based approach to identifying at‑risk residents and implementing timely, interdisciplinary interventions that can help prevent avoidable hospital transfers and functional deterioration. Participants will learn how to recognize early warning signs, including acute changes of condition, and apply effective clinical and evidence-based strategies to improve communication and elevate resident care. Participants will explore cross‑department collaboration and documentation practices that strengthen clinical reasoning and support quality outcomes. The focus is on practical, real‑world processes that centers can implement immediately to improve resident stability and prevent decline and hospitalizations and/or hospital readmissions.

Presenters: Allison Spangler and Felicity Wood, both with Health Quality Innnovators
