    With over 70% of the U.S. population having survived some kind of traumatic event, a health care provider’s ability to appropriately care for patients with a history of trauma is critical. This virtual, live training will will provide attendees with the tools to develop emotionally secure environments for residents in their care.     Skilled nursing center surveys can be overwhelming; however, proper planning and preparing your team throughout the year can make for a successful survey. Centers can do this by maintaining a state of readiness, making advanced and ongoing preparations, and training staff to understand their role when it comes to interacting with surveyors. This virtual training will support your team’s understanding of the before, during and after-survey process and help instill confidence in working with surveyors and responding effectively if questions or issues arise. Attendees will hear from the Agency for Health Care Administration and other long term care experts, who will share the information needed to gain knowledge and an understanding of how to lead their team to better understand their roles, while also reviewing best practices for successful survey outcomes.

    Upon completion, participants will be able to:

    • Discuss the types of survey and the regulation requirements that must be met.
    • Review the survey process – anticipating survey visits, what documents/materials/reports are required immediately, steps in the event of a citation, post-survey evaluation and corrections if needed.
    • Examine the keys to internal preparation and considerations for staff and outline best practices to ready staff to respond to the arrival of survey teams.
    • Address responding to a citation and CMS guidelines that identify a potential Immediate Jeopardy citation.

    FHCA’s Introduction to Risk Management program equips participants with the essential tools to establish or strengthen their nursing center’s risk management strategies. Participants will learn about critical topics such as advance directives, Florida’s state-specific risk requirements and relevant federal regulations and gain insight into the reporting standards, investigation steps, QA and QAPI, and documentation required by both state and federal guidelines. Participants will gain a greater understanding of high-risk areas, including elopement, significant incidents, and accidents, while also learning how to prevent a comment or concern from turning into a complaint. The valuable training is led by industry experts, including representatives from the Agency for Health Care Administration, RB Health Partners, Inc., Attorney Karen Goldsmith and other risk managers from FHCA’s Risk Compliance Council, and designed for administrators, risk managers, Directors of Nursing and other clinical leaders, and Social Service Directors.

    Karen Goldsmith is Florida Health Care Association's Regulatory Counsel and previously served as the association's Legal Counsel since 1980. She represents several providers of nursing homes and assisted living facilities. She is active on the American Health Care Association Legal Committee and served as its chairman for two terms. She is active in the American Health Lawyers Association. She speaks often on health care and legal issues and has written numerous articles.

    Michelle Dillehay BSN, RN, GERO-BC, has been employed with the Agency for Health Care Administration since 1999. Currently, she enjoys working in the Survey and Certification Support Branch to provide training and quality assurance in Field Operations. She served as a supervisor and preceptor for the Orlando office that includes Orange, Seminole, Brevard and Osceola counties. Michelle was also a Registered Nurse Specialist surveyor and a Quality-of-Care Monitor for nursing homes and assisted living facilities from Daytona Beach to Miami. Michelle received her Bachelor of Science in Nursing from Kent State University in Ohio in 1988 and has her Certification in Gerontology from the American Nurses Credentialing Center. She has over 30 years of experience in long term care.

    Brandi Byrd has been with Agency for Health Care Administration since January 2016. She is a General & Operations Manager for the Complaint and Intake Management Unit within the Division of Health Care Policy and Oversight. The unit oversees every facility reported incident submitted to the Agency from multiple providers, including hospitals, nursing homes, skilled nursing facilities, assisted living facilities, ambulatory surgical centers, and abortion clinics. Prior to her current role, she was a surveyor with Agency for several years. Ms. Byrd is a Registered Nurse with 15 years of professional experience providing direct care and risk management oversight for multiple provider types.

She is a risk manager and for six years was the administrator of a 68-bed assisted living facility with an Extended Congregate Care license. She has recently received her Certification in Infection Control (CIC) as well. She came to FHCA in May of 2017 and is the liaison to the Senior Clinicians Council, Risk Compliance Council, Infection Prevention Council, and the Florida Center for Assisted Living (FCAL) Committee.

    Hazel Mahoney is Executive Risk Compliance Advisor at RB Health Partners, Inc. She has been a nurse since 1975 and remains excited about working in health care. Throughout her career, she has worked in acute care in nursing, case management and risk management before coming to long term care in approximately 2000. Since that time Hazel has served three companies in risk roles adding compliance in 2018. During Hazel's long term care career, she has engaged in risk and compliance-related committees, subgroups and the promotion of professionals pursuing these important areas. She has been the Chair of the Florida Health Care Association Risk and Compliance Counsel and remains active and in a leadership role today. Hazel has experience in guiding facilities under corporate integrity agreements through the Office of Inspector General (OIG).  Hazel is well known for her skills in developing risk and compliance programs, education and training and mentoring those who wish to further develop in these areas.     Life Safety code compliance and survey preparation are a crucial part of the health and well-being of skilled nursing center residents and staff.     Eddie Alday has been with the Agency for Health Care Administration since December 2012. Currently, Eddie is the Bureau of Field Operations Life Safety Code Lead. He is responsible for providing oversight, instruction and development of quality assurance activities for the life safety code program within the Bureau of Field Operations. Eddie served 24 years in the US Army as a Combat Engineer; he has degrees in Business and Construction Management.     John Altherr joined the Agency for Health Care Administration (AHCA) in September 2013. Currently he serves as a Life Safety Code Lead within the Division of Health Quality Assurance. As the Agency’s Life Safety Code Lead, he is responsible for providing oversight, instruction and development of quality assurance activities for the Agency’s Life Safety Code Survey program. Prior to joining AHCA, Mr. Altherr was a General Manager with Northwest Airlines Inc., ground operations division and retired after 20-years of service.     Clinton Slier is a Life Safety and Environment Specialist and has been with Integrity Health Care Systems since August of 2017.     This second installment of A Proactive Approach to Elopement Prevention will emphasize a proactive framework to reduce elopement risk, focusing on early identification, assessment, and care planning to minimize potential incidents.

    This second installment of FHCA's Proactive Approach to Elopement Prevention is a two-hour training featuring members of FHCA's Risk Compliance Council who will emphasize a proactive framework to reduce elopement risk, focusing on early identification, assessment, and care planning to minimize potential incidents. Participants will also explore real-world scenarios involving near misses and actual elopements, gaining insight into appropriate and effective provider responses. Participants will leave with actionable knowledge on evaluating resident care needs, implementing individualized safety plans, and strengthening response protocols—all with the goal of promoting resident safety and enhancing quality of care.

    Learner Objectives

    Upon completion, participants will be able to:

    • Describe three elopement evaluation and assessment strategies
    • Discuss methods to engage staff to identify potential change-in-condition that may result in the need for reevaluation and/or assessment

    Karen Goldsmith is Florida Health Care Association's Regulatory Counsel and previously served as the association's Legal Counsel since 1980. She represents several providers of nursing homes and assisted living facilities. She is active on the American Health Care Association Legal Committee and served as its chairman for two terms. She is active in the American Health Lawyers Association. She speaks often on health care and legal issues and has written numerous articles.

    Mary Maloney

    Mary Maloney is the Chief of the Bureau of Field Operations in the Agency for Health Care Administration.  She has been employed at the Agency for over 35 years.  Mary is a Registered and Florida Licensed Dietitian/Nutritionist and board certified as a Gerontological Nutrition Specialist.  She is based out of the Clearwater office and her agency experience includes conducting surveyor training, quality improvement activities, assisting with Agency legislation, and health facility surveys.  Her expertise is nursing home regulation, as well as hospitals, assisted living facilities, and intermediate care facilities for individuals with intellectual disabilities.  Mary earned her master’s degree in public health with emphasis in health education and public health nutrition from University of South Florida.  She has held numerous leadership and board positions with district and state dietetic associations.  She has also given many presentations to a variety of professional organizations and co-authored professional journal articles and textbooks.  She enjoys crafts, learning foreign languages, and cooking international cuisine in her leisure time.

Michelle was also a Registered Nurse Specialist surveyor and a Quality-of-Care Monitor for nursing homes and assisted living facilities from Daytona Beach to Miami. Michelle received her Bachelor of Science in Nursing from Kent State University in Ohio in 1988 and has her Certification in Gerontology from the American Nurses Credentialing Center. She has over 30 years of experience in long term care.

She received her undergraduate and graduate degrees at Florida State University in Speech Pathology. Robin’s career has focused on post-acute care initially in Therapy Services.  Robin then became a Healthcare Risk Manager and focused her efforts on proactive and response-driven Risk Management. Robin is the Chair of the FHCA Risk and Compliance Council.

    Kim Broom

    Kim Broom is FHCA Director of Clinical & Risk Management. She has been a Registered Nurse for 38 years, with 27 years in long term care as a Director of Nursing, Regional Nurse Consultant and other roles. She is a risk manager and for six years was the administrator of a 68-bed assisted living facility with an Extended Congregate Care license. She has recently received her Certification in Infection Control (CIC) as well. She came to FHCA in May of 2017 and is the liaison to the Senior Clinicians Council, Risk Compliance Council, Infection Prevention Council, and the Florida Center for Assisted Living (FCAL) Committee.

    Tracy Greene

    Chief Operating Officer

    Tracy Greene began her career in healthcare as a Certified Therapeutic Recreation Specialist at an inpatient psychiatric hospital in the Florida panhandle. She then moved to the Florida Keys and worked in long-term care as a Social Services and Admissions Director, eventually moving into an Assistant Administrator role. She became a Nursing Home Administrator in 2000 and was also the company’s internal Emergency Preparedness Coordinator. In this role, she worked collectively with the CEO, COO, and the County to streamline disaster planning for three nursing homes. This included obtaining CDL Class B licenses for several employees (including herself) and managing the use of school buses.

    Tracy moved to Tampa approximately five years later and accepted a position with Southern Health Care Management (SHCM) as an Administrator. Later she was promoted to Sr. Administrator, then Vice President of Operations. She worked for SHCM for a total of 13 years where she won Administrator of the Year. In addition, Tracy was an Administrator for Woodbridge Care Center in Tampa for four years, as well as a Regional Vice President for Life Care Centers of America for two years.  

    Tracy is an active member of the Florida Health Care Association (FHCA) and has been on FHCA’s Emergency Preparedness Committee since 2005, serving as the Vice Chair and most recently the Chair. Tracy has testified in front of the Florida Legislature, publicly spoken on emergency preparedness at the Governor’s Hurricane Conference and presented at FHCA Annual Conferences. 

    When not working, you can find Tracy watching sporting events, especially football, enjoying time out on the ocean, doing Pilates, traveling, and participating with her alma mater, University of Florida.
